The Diocese of Guildford will follow the NCIs Family Friendly Policies document provided by the NCI which includes Parental Pay and Leave Advice

Parental Leave Form

The Parental Leave Form is to be completed in all cases of Parental leave (Maternity, Paternity, Adoption and Shared Parental leave). The diocese also need all relevant documents and correspondence from the payee, i.e. MATB1, SC3, shared parental leave form and letters detailing intention. In line with statutory requirements, it is essential that these forms are sent to the diocese at least 28 days prior to the leave commencing.
